Prevent mishaps at home and plan a response in the event of an emergency with our Proactive Planning service:
We will conduct a home safety evaluation and identify modifications to prevent falls and accidents before they happen and make your loved one feel safe at home.
Then, we’ll gather all medical and legal documents needed to respond to an emergency and design a crisis management strategy so you’ll have the comfort of knowing there is a plan in place should the unlikely happen.

Complete Plan of Care
We’ll develop and execute an individualized, Complete Plan of Care so you and your loved ones can relax and focus on creating happy memories together.
We’ll perform a comprehensive assessment of the medical, functional, emotional, social and environmental components that influence senior health and well-being.
Next, we’ll prepare recommendations and meet with you and your family to decide the best plan of care, tailored to your needs.
We’ll assign a Geriatric Care Manager to coordinate and manage all of the services and resources needed to make daily life easier, and check in regularly to make sure that all your needs are met.
And, we’ll be available by phone 24 hours a day, 7 days a week to respond to emergencies and health crises.
Our Complete Plan of Care is billed by the hour. We typically spend more time at the beginning of our work together as we make assessments and prepare recommendations. Our billable hours decrease significantly once we have a plan of care in place. Contact us to get started.
Meet our Management Team
Cindy Tulimieri, OTR/L, ECHM, CDP, President
Cindy has worked with the geriatric population for over 20 years, many of them as a manager in a skilled nursing rehabilitation setting. She founded ILI Therapy Staffing in 2011 and in 2018, she launched Living Innovations Care Management to help keep seniors in their homes or preferred place to live and support a vibrant life in that space. Cindy holds a Bachelor of Science degree in Occupational Therapy from the University of New England, and is certified by the National Board for Certification in Occupational Therapy (NBCOT) and licenced in the Commonwealth of Massachusetts. She has completed an Executive Certificate of Home Modification from USC Leonard Davis School of Gerontology, is a certified Dementia Practitioner, and serves as a board member of the Bedford, Massachusetts Council on Aging. In her spare time, she enjoys fostering dogs until they find their forever homes, gardening and travelling as much as possible.
Personal experience drives Cindy’s passion for working with people with disabilities. When an aunt suffered a severe stroke at 60 and lost the ability to speak and use her right side, Cindy took her into her home and experienced first hand how overwhelming it can be to care for a loved one at home—and then how hard it was to transition her to a nursing home when she couldn't care for her any longer. As a result, Cindy encourages every member of her staff to treat each client as they would their own family member.
Faith Zellner COTA/L, CMC, CDP, Vice President
Prior to joining LI Therapy Staffing and Living Innovations Care Management in 2017, Faith spent 10 years as an Occupational Therapy Assistant at a continuous care retirement community and spent a few years working as a private Geriatric Care Manager. Faith holds a Bachelor of Arts degree in Social Rehabilitation Services from Assumption College and continued her education to become a Certified Occupational Therapy Assistant (COTA). Faith is a Certified Care Manager (CMC) by the National Academy of Certified Care Managers (NACCM). She is a proud member of the National Board for Certification in Occupational Therapy (NBCOT), the Aging Life Care Association® (ALCA), and is a certified Dementia Practitioner. In her spare time, Faith enjoys spending time with her family and young children, trying to keep up with their busy lives.
